This form is used by students who wish to grant access to their Flagler College Academic Record to an identified individual or multiple individuals. The form is used specifically for each Academic Year and must be completed and submitted each Academic Year (September 1 - August 31). Individuals specified on form must produce proper identification for access to Academic Records.
This form grants identified individuals NO MORE access to the Academic Record than the student him-or herself has.

This form is necessary for pre-registering for a course that requires instructor permission and also for students to submit if they have not completed a pre-requisite, but the instructor is willing to waive the pre-requisite to enter the course. The submission of a signed form only provides the student with the ability to pursue getting into the course on a SPACE AVAILABLE basis.
Fill this 2 page form and questionnaire out and return it to the Office of the Registrar if you have chosen not to return to Flagler College for the upcoming semester.
